		All new patients must fill out the Patient Information form. If you are a new GYN patient please also fill out the Gynecology form, and patient information. If you are a new OB patient please fill out The Pregnancy Consent, Pregnancy Questionnaire, and Patient Information. Completing these forms and sending them prior to your appointment can reduce your wait time. You may also bring the completed forms to your appointment.
